Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse for a remote position. You will extend care by addressing patients' medical, social, and psychological needs via phone, with responsibilities including documenting calls, scheduling appointments, and arranging transportation.

Qualifications

• Active, non-probationary state RN license in all OSH practice states.
• Ability to manage time and work autonomously with reliability.
• Excellent communication and customer service on every call.
• Willingness to work flexible shifts including evenings, weekends and holidays.

Responsibilities

• Provide confident and professional service to OSH patients on calls.
• Follow current Prescription Refill policy and document interactions.
• Document accurately during and after each call.
• Schedule appointments and arrange transportation as needed.
• Monitor CareReqs and complete tasks within scope.
• Support the CareReq queue and meet departmental metrics.
• Liaise with patients to ensure off-cadence care and follow-up.
