Oak Street Health is hiring a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients' medical, social, and psychological needs via telephone.

Qualifications

• Active, non-probationary RN license in all OSH practice states.
• Proactive maintenance of licenses without prompt or audit.
• Ability to manage time and work autonomously with reliability.

• Willingness to work flexible shifts outside core hours.
• US work authorization.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current prescription refill policy.
• Document clearly during calls and complete all patient requests.
• Check the CareReq queue and ensure CareReqs are complete.
• Schedule appointments and arrange transportation for clinic visits.
• Meet departmental metrics and participate in ongoing responsibilities.
• Other duties as assigned.
