Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care by addressing patients' medical, social, and psychological needs over the phone, outside typical clinic hours. In this remote role, you will provide professional support to patients, document calls accurately, schedule appointments, arrange transportation, and help ensure timely care.

Requirements include an active RN license in all OSH practice states, ability to work autonomously, flexible shifts, and US work

Qualifications

• Active, non-probationary state Registered Nurse license in all states of OSH practice.
• Ability to work autonomously with reliability and strong communication.
• Willingness to work flexible shifts including evenings, weekends, and holidays.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Record clear and precise documentation while on a call.
• Schedule appointments for patients and arrange transportation as needed.
• Check the CareReq queue and ensure CareReqs are complete.
• Complete all tasks within scope and ability of the role.
