Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse for a remote role. You will extend care by telephonically addressing patients’ medical, social, and psychological needs, supporting OSH patients outside typical clinic hours.

The position requires an active RN license in all OSH practice states, US work authorization, and the ability to work flexible shifts, including evenings, weekends, and holidays. Competitive benefits and a comprehensive pay range are provided.

Qualifications

• Active, non-probationary RN license in all states of OSH practice.
• Ability to manage time and work autonomously with reliability.
• Flexibility to work evenings, weekends and holidays.
• US work authorization required.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Schedule appointments for patients and arrange transportation as needed.
• Document clearly and completely during calls; ensure all requests are completed.
• Handle tasks within the scope of the role and contribute to team metrics.
