Job summary

Vail Health Hospital is seeking a Triage RN to perform care coordination and triage nursing duties, supporting providers in triaging patient calls and portal messages. You will document assessments and plans in the electronic health record, participate in KPI huddles, and ensure compliance with HIPAA/OSHA while coordinating care pathways.

The role requires 3 years of clinical experience and strong nursing assessment skills, with licensure for Colorado or a valid compact state.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Triage calls to assess severity and route to appropriate care.
• Document triage assessments and plans in the EHR.
• Participate in weekly KPI huddles and coordinate with providers.
• Maintain HIPAA-compliant workflows and patient confidentiality.
• Support CPC+ Care Coordination team processes.
• Ensure proper transfer of pertinent medical information between departments.
