Job summary

Qualifications

• Knowledge of disease processes and patient assessment across age groups.

Responsibilities

• Evaluate and triage patients using established protocols.
• Support providers in triage by phone and portal messages.
• Document assessments, plans, and patient responses in EHR.
• Coordinate same-day and future appointments and manage refills as per policy.
• Participate in care coordination teams and quality improvement activities.
