Job summary

Vail Health is seeking a Triage RN to perform care coordination and triage nursing. You will assess health concerns, triage to emergency, urgent care or clinic visits, and support providers by prioritizing patient needs.

Responsibilities include documenting in the EHR, reviewing KPIs in weekly huddles, and ensuring continuity of care. A Colorado RN license or compact license is required, with 3 years of relevant experience preferred.

Qualifications

• Three years of clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with or without basic visual inspection.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules.

Responsibilities

• Triage and assess patient health concerns using established protocols and guidelines.
• Support care coordination by triaging calls and portal messages.
• Document assessments, plans, and outcomes in the EHR with appropriate follow-up.
• Participate in weekly huddles to review KPIs and coordinate with providers.
