Job summary

Vail Health seeks a Triage RN to perform care coordination and triage for emergency, urgent care, and clinic visits. You will support providers by triaging patient calls and portal messages, and document assessments and plans in the EHR with accuracy.

Ideal candidates have 3+ years of clinical nursing experience, and hold a Colorado RN license or compact-state license. The role includes collaboration in a care coordination team and adherence to HIPAA and OSHA standards.

Qualifications

• 3 years of clinical experience in a nursing role preferred.
• State RN license required (Colorado or compact state).
• Experience with triage, care coordination, and documentation in EHR.

Responsibilities

• Assess severity of health concerns using triage protocols.
• Support providers in triaging calls and portal messages.
• Document symptoms, assessments, and plans in the EHR.
• Participate in weekly huddles to review KPI.
• Coordinate care to emergency, urgent care, or clinic appointments.
• Follow policies to improve access and outcomes.
