Job summary

Vail Health Hospital is seeking a Triage RN to perform care coordination and triage nursing tasks. You will assess health concerns using protocols and support providers with triage calls and portal messages within licensed practice guidelines.

You will document assessments, coordinate appointments and follow protocols to improve patient access to care, while contributing to quality improvement initiatives and patient education.

Qualifications

• 3 years clinical experience with patient care and admin duties preferred.
• Knowledge of disease processes and development across age groups.
• Nursing assessment skills and basic theory in practice.
• Understanding of medical terminology, HIPAA compliance and OSHA rules.

Responsibilities

• Evaluate and triage patient care needs using established protocols.
• Support providers in triage calls and portal messages.
• Document symptoms, assessments, advice and patient responses.
• Schedule same-day and future appointments on provider schedules.
• Follow standing orders and triage algorithms; manage refills per protocol.
• Participate in CPC+ care coordination and performance improvement.
• Provide patient education on chronic disease, prevention and family health.
