Qualifications

• BSN preferred and RN licensure in SC with compact privileges.
• 3+ years of acute care clinical nursing experience.
• Experience providing education and support for chronic condition management.
• Experience with community resource coordination and referrals.

Responsibilities

• Conduct telephonic assessments to identify clinical, behavioral, social, and environmental needs.
• Identify and resolve barriers to effective care and treatment adherence.
• Develop, implement, and update individualized care plans.
• Monitor progress toward outcomes through ongoing assessment.
• Provide education and support for chronic condition management and self-care.
• Coordinate care and facilitate access to providers, services, and community resources.
• Collaborate with members, families, providers, and interdisciplinary teams.
• Maintain timely, accurate documentation per guidelines and compliance.
• Apply department and organizational policies in day-to-day work.
• Exercise independent judgment in managing assigned caseloads.
