Job summary

Humana Inc. is seeking a Telephonic Nurse - Care Manager to support patients transitioning into and out of CenterWell Home Health services. This remote, part-time role (20 hours/week) requires an RN license, an associates degree, and 3 years of post-acute nursing experience.

You will assess needs, educate patients, and coordinate post-acute care options by phone. The position emphasizes empathy, clear communication, and collaboration with clinical teams.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license; may require additional state licensures.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong analytical, problem-solving, and empathetic communication skills.

Responsibilities

• Support patients discharged from service or needing post-acute services as a telephonic RN.
• Educate patients on post-acute care options, Medicare coverage, and resources.
• Determine level of care, match to service sites, and facilitate referrals.
• Assist admissions and ensure timely responses and patient satisfaction goals.
• Adhere to HIPAA, ethics, and company policies; participate in special projects.
