Job summary

Humana, Inc. (CenterWell Home Health) is seeking a Telephonic Nurse Care Manager to support patients transitioning into and out of post-acute services from a remote, home-based setting.

The role focuses on coordinating care across home health, hospice, and palliative options and ensuring timely follow-up and patient advocacy. You will work 20 hours weekly, Saturdays and Sundays 10am–8:30pm EST, with weekday training evenings and occasional field/office travel.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license; additional state licenses may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Learn and master information related to locations and services of clients.
• Analytical and can problem-solve.
• Excellent verbal and interpersonal skills.
• Communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients who have been discharged from service or may need post-acute services.
• Navigate healthcare options; care services post-acute offerings, Medicare coverage, billing issues, and accessing healthcare resources.
• Make clinical level of care determinations based on discussion, medical records and other data.
• Match needs to service site location or provide alternatives; advocate during referrals.
• Follow up and track referral and admission outcomes.
• Adhere to department performance objectives and patient satisfaction goals.
• Assist in admissions by acting as an ambassador for patients meeting requirements.
