Job summary

Humana Inc, CenterWell Home Health, is seeking a Telephonic Registered Nurse Care Manager for a part-time, remote role. You will support patients transitioning from discharge, assess post-acute needs, and coordinate home health, hospice, and palliative care services.

The role requires an Associate's degree (BSN preferred), a compact RN license, and at least 3 years post-acute nursing experience. Training evenings/weekends and occasional travel may be needed.

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service and those needing post-acute care.
• Provide patient options and education on post-acute care and related services.
• Make clinical care determinations and match needs to service sites; advocate for timely referrals.
• Follow up and track referral and admission outcomes.
• Assist admissions and placement to appropriate care settings.
