Humana Inc. in CenterWell Home Health is seeking a Telephonic Nurse Care Manager to support patients transitioning in and out of services. This remote, part-time role requires weekend shifts and weekday training, with a 20-hour schedule and a focus on post-acute care coordination.

You will assess needs, determine appropriate post-acute options, educate patients, and liaise with clinical teams. A compact RN license, an Associate degree, and at least three years of post-acute nursing experience

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license required.
• At least 3 years of post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Ability to learn locations and services of clients.
• Analytical and problem-solving skills.
• Excellent verbal and interpersonal skills with empathetic phone communication.
• Willingness to travel occasionally to Humana offices for training or meetings.
• Remote work readiness and ability to work in a telephonic care role.

Responsibilities

• Support patients discharged from service by navigating post-acute care options.
• Provide patient education on post-acute services and Medicare coverage.
• Determine clinical level of care based on data and match to service sites.
• Coordinate referrals and assist admissions while focusing on timely responses.
• Maintain alignment with department performance goals and patient satisfaction.
