Humana Inc. is seeking a part-time Care Manager, Telephonic Nurse, to support patients transitioning into and out of CenterWell Home Health services.

This remote, telephonic role requires a compact RN license, an associate degree (BSN preferred), and a minimum of three years in post-acute nursing, including home health or hospice experience. The position involves weekend scheduling, patient education, and collaboration with the clinical team to optimize care plans and admissions.

Qualifications

• Associates Degree required. BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong analytical and problem-solving abilities.
• Excellent verbal and interpersonal skills.
• Empathetic phone communication.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service or needing post-acute services.
• Provide patient options and education using multiple tools; handle location lookups and inquiries.
• Make clinical level of care determinations based on discussions and records.
• Assist in admissions and act as an advocate to maximize timely referrals and admissions.
• Ensure department performance objectives and patient satisfaction goals are met.
• Educate patients and families about Home Health, Hospice, and Palliative Care Services.
• Participate in special projects and other responsibilities as assigned.
