Humana Inc. seeks a Telephonic Registered Nurse Care Manager to work from home, guiding patients through post-acute care options and coordinating with the CenterWell Home Health clinical team. The role focuses on ensuring continuity of care and timely referrals while upholding HIPAA and company policies.

The position is part-time (20 hours/week), with Saturday-Sunday 10am–8:30pm EST and weekday training options in evenings/weekends. Travel to Humana offices may be required occasionally.

Qualifications

• Associates degree required; BSN preferred.
• RN license in good standing; compact state license may be required.
• At least 3 years of post-acute nursing experience; home health or hospice preferred.

Responsabilités

• Be a CenterWell Home Health representative supporting patients discharged from service or needing post-acute services.
• Provide patient education and options using various resources and sites of service.
• Make clinical level of care determinations based on discussions and records; match needs to services.
• Assist admissions by acting as ambassador for patients meeting admission requirements.
• Ensure patient satisfaction goals are met and follow up on referrals and admissions.
