Humana Inc. through CenterWell Home Health is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning through post-acute care in a remote setting.

This part-time role schedules 20 hours per week with Saturday and Sunday shifts from 10am–8:30pm EST and weekday training in the evenings or weekends. You will assess clinical needs, coordinate post-acute services, assist admissions, and educate patients and families by phone while upholding HIPAA privacy and company

Qualifications

• Associates degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills; ability to communicate with empathy over the phone.

Responsabilités

• Support patients recently discharged or needing post-acute services; navigate care options and resources.
• Provide patient options and education on sites of service, post-acute care, and related questions.
• Determine clinical level of care based on discussion, records and data; refer to appropriate service site.
• Assist admissions and act as ambassador for patients meeting admissions requirements.
• Adhere to department performance objectives and patient satisfaction goals.
