Humana Inc. is seeking a Telephonic Registered Nurse to support patients remotely as a Care Manager.

This part-time role (20 hours/week) coordinates post-acute care and guides patients through home health, hospice and palliative services in collaboration with the CenterWell clinical team. Candidate must have an associates degree (BSN preferred), an RN license, at least 3 years in post-acute nursing, and excellent phone-based communication.

Qualifications

• Associates Degree required; BSN preferred.
• RN license (compact state) with possible additional state licenses.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong communication and empathy over the phone.

Responsabilités

• Support patients discharged from service with post-acute options and education.
• Provide patient options and education on post-acute care and service options.
• Make clinical level of care determinations and match needs to service sites.
• Follow-up and track referral and admission outcomes.
• Assist admissions by acting as an ambassador for patients meeting requirements.
• Contribute to patient placement accuracy and timely care transitions.
