Job summary

Humana Inc. is seeking a Telephonic Nurse Care Manager to support patients transitioning between home health, hospice, and post-acute services in a remote capacity.

The role requires an RN with at least 3 years post-acute experience, an Associates degree (BSN preferred), and the ability to work Saturdays and Sundays 10am–8:30pm EST. Training evenings on weekdays are included. This is a 20-hour weekly part-time position.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health/hospice preferred.
• Knowledge of home health, hospice, and palliative care.
• Excellent communication and empathy over the phone.
• Ability to navigate post-acute care options and resources.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients who have been discharged from service or for those who may need post-acute services.
• Navigate healthcare options and provide patient education on post-acute offerings and Medicare coverage.
• Make clinical level of care determinations and match needs to service sites or alternatives.
• Follow-up and track referral and admission outcomes.
• Assist in admissions by acting as an ambassador for patients meeting requirements.
• Adhere to company policies and participate in mandatory training.
