VSHP Volunteer State Health Plan, Inc is seeking a Registered Nurse Case Manager to advocate for the health and well-being of members with intellectual and developmental disabilities, coordinating long-term services under TennCare and related benefits.

The role requires monthly in-person visits and telephonic contacts, TN RN license, 5 years healthcare experience (3 years clinical), and willingness to travel in Hamblen or Greene Counties. BCBST driver program may apply.

Qualifications

• Active RN license in Tennessee required.
• Minimum 3 years clinical experience in healthcare.
• Total 5 years in healthcare, with IDD experience (2 years for Select Community).

Responsabilités

• Support utilization management for complex cases.
• Serve as liaison between members, providers and internal/external customers.
• Oversee highly complex cases to ensure appropriate interventions.
• Perform essential case management activities: assessment to evaluation.
• Digital communications capability and tech support readiness.
• Commitment period of 18 months before posting to other internal roles.
• Possible immunizations or medical tests as required.
