VSHP Volunteer State Health Plan, Inc is seeking a Registered Nurse Case Manager who will assess members, coordinate long-term services and supports, and develop integrated care plans. The role requires telephonic contact and monthly in-person home visits within the Tennessee service area.

Candidates must hold an active RN license in Tennessee, have 5 years of healthcare experience with 3 years in a clinical setting, and be willing to travel to Hamblen or Greene County, TN.

Qualificações

• Active RN license in Tennessee.

Responsabilidades

• Support utilization management for complex cases.
• Liaise between members, providers and internal/external customers.
• Oversee complex cases to ensure effective interventions and benefits utilization.
• Perform assessment, planning, implementation, coordination, monitoring and evaluation.
