Job summary

Stella Healthcare – Fully Remote seeks a compassionate Social Worker (BSW/MSW) to provide remote psychosocial support, care coordination, and resource navigation for patients in chronic care programs.

You will assess needs via phone/video, develop care plans, connect to resources, document in the EMR, and collaborate with nurses and care managers while ensuring HIPAA compliance. This is a fully remote role with flexible scheduling.

Qualifications

• Bachelor’s or Master’s Degree in Social Work (BSW or MSW) or related field.
• Healthcare, chronic care management, or case management experience preferred.
• Strong clinical assessment and communication skills.
• Comfortable working independently in a fully remote environment.
• Proficiency with EMR systems and video conferencing tools.

Responsibilities

• Conduct comprehensive psychosocial assessments via phone or video.
• Develop individualized care plans addressing medical, emotional, and social needs.
• Connect patients and families to community resources, financial assistance, and healthcare services.
• Provide emotional support, short‑term counseling, and crisis intervention as needed.
• Assist patients in navigating insurance, transportation, housing, and food insecurity challenges.
• Collaborate with nurses, care managers, and primary care providers.
• Document all patient interactions accurately in the EMR.
• Maintain HIPAA compliance and organizational policies.
• Stay informed on local and national social service resources and eligibility requirements.
