Job summary

Qualifications

• Active professional licensure in one of the listed fields.
• Must reside in one of the specified states or eligible to work in the United States.
• Two years of experience with geriatric behavioral health or older adults preferred.
• Strong communication, assessment and organizational skills.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Conduct basic psychosocial assessments to identify needs and supports.
• Coordinate with care team to develop individualized plans.
• Connect patients with community resources and programs.
• Educate patients and caregivers on navigating healthcare systems.
• Advocate for patient goals and ensure care aligns with preferences.
• Coordinate with providers to promote care continuity.
• Document interactions and interventions in the EHR.
• Monitor progress and follow up on needs.
• Flag urgent psychosocial or behavioral concerns to the team.
