Job summary

TeleMate Health INC. is seeking a TeleMate Health Social Worker to support patients’ health and wellness through resource coordination, education, and advocacy. This fully remote role collaborates with a care team to overcome barriers to care and improve quality of life from a home setting.

Ideal candidates hold an active LPN or RN license and reside in one of six states. Two years in geriatric behavioral health or chronic disease management is preferred.

Qualifications

• Active nursing license (LPN or RN) in the US.
• Must reside in AL, GA, KY, MS, PA, or TN.
• 2+ years in geriatric behavioral health or chronic disease management preferred.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Perform psychosocial assessments to identify patient needs.
• Develop individualized care plans with the care team.
• Link patients to community resources and benefits.
• Educate patients and caregivers on navigating care.
• Document encounters in the EHR.
• Monitor progress and follow up on needs.
• Escalate urgent psychosocial or behavioral concerns.
