Formación

• Bachelor’s or Master’s Degree in Social Work (BSW or MSW) or related field.

Position Overview

The Social Worker will provide remote psychosocial support, care coordination, and resource navigation for patients enrolled in chronic care management programs. You will serve as an advocate and problem-solver, helping patients overcome barriers that impact their health and well-being.

This position requires strong communication skills, clinical judgment, and the ability to work independently while maintaining high standards of professionalism and documentation.

Key Responsibilities
