Qualifications

• Bachelor's degree in Social Work (BSW) required.
• Active social work license.
• Minimum 2–5 years of experience in healthcare, care management, care coordination, or community-based social work.
• Experience addressing SDOH in chronic care, value-based care, or population health settings preferred.

Responsibilities

• Assess patients' social, emotional, environmental, financial, and behavioral health needs using motivational interviewing and SDOH assessments.
• Meet with patients to discuss identified needs and develop personalized care plans.
• Coordinate referrals to internal and external resources and ensure closed-loop referrals.
• Educate clients and families about available resources and services.
• Document assessments, interventions, and referrals in the EHR.
• Support audits, reporting, and quality improvement initiatives.
