Job summary

Devoted Health is seeking a medical social worker to support high-needs patients remotely in a fast-paced, collaborative setting. You will engage with patients to address social barriers, coordinate care across interdisciplinary teams, and assist with end-of-life discussions.

A Master’s in Social Work with an active license is required, with Spanish bilingualism preferred. Responsibilities include conducting screenings, creating individualized action plans, and supporting caregivers.

Qualifications

• 3+ years working in medical social work, case management, or care coordination
• Master’s degree in social work and active state license
• Bilingual in Spanish preferred
• Health insurance experience preferred (Medicare Advantage a plus)
• Experience with senior population a plus
• Proven ability to build relationships with patients and interdisciplinary teams
• Comfortable with working remotely
• Ability to multi-task while listening, talking, evaluating and writing notes

Responsibilities

• Establish trust and build relationships with patients telephonically and virtually
• Collaborate with interdisciplinary team to ensure cohesive, patient-centered care for high-needs patients
• Identify social barriers to assist in resource alignment and coordination
• Conduct screenings and create personalized action plans
• Recognize caregiver burnout and provide support/resources
• Assist in navigating complex family/support issues
• Facilitate conversations about end-of-life planning
• Coordinate care and monitor outcomes with Behavioral Health Team
• Balance multiple priorities for complex patients
• Build relationships with local market and internal stakeholders
