Job summary

Devoted Health is seeking a Social Work Case Manager to provide remote social work case management for our Primary Care program. You will support psychosocial needs, coordinate with an interdisciplinary team, and help deliver high-quality care to home-based patients with chronic conditions.

The role emphasizes telephonic/video visits, strong patient relationships, and collaboration with clinicians to ensure cohesive, patient-centered care in a fast-paced virtual environment.

Qualifications

• Master’s degree in social work and active license.
• Minimum of 2 years expertise in the medical field.
• Comfortable with technology and a fast-paced environment.
• Self-starter with ability to succeed in a work-from-home environment.
• Strong interpersonal and communication skills.
• Comfortable working with patients over video and phone.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Establish trust and build strong relationships with patients.
• Collaborate with clinicians, nurses, and care coordinators for cohesive care.
• Conduct Social Work Case Management interventions to maximize program effectiveness.
• Provide psychosocial screenings with personalized care plans.
• Offer emotional and caregiver support and navigate family/caregiver dynamics.
• Document all clinical interactions in the electronic health record.
