Responsibilities

• Identify patients at high risk for complications and coordinate care with the care coordination team.
• Develop and implement strategies to improve patient health and quality of life outcomes.
• Refer and coordinate services to ensure cost-effective provision of physical health, behavioral health, and home/community services.
