Job summary

Devoted Health is seeking a Remote Primary Care Social Work Case Manager to support our Primary Care program. You will perform telephonic and video visits, build trusting relationships with patients and caregivers, and collaborate with clinicians to deliver holistic, patient-centered care from home.

The role requires an MSW with an active license and at least 2 years in healthcare, plus comfort with technology and fast-paced environments. Remote work in the U.S. is supported.

Qualifications

• MSW degree with active license.
• Minimum 2 years of medical field experience.
• Comfortable with technology and telemedicine (video/phone).
• Strong interpersonal and communication skills.
• Proficient with remote work tools.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Establish trust and build relationships with patients.
• Collaborate with clinicians, nurses, and care coordinators for cohesive care.
• Conduct social work case management interventions and screenings.
• Develop personalized care plans and address social determinants of health.
• Document all clinical interactions in the EHR.
• Coordinate with local market stakeholders and participate in team activities.
