Job summary

Integrated Sleep Care, a Pivotal Health Company, seeks a Remote Sleep Care Coach to support patients on CPAP therapy, providing education, motivation, and ongoing remote guidance to improve sleep and health outcomes.

The role focuses on coaching, care coordination, and delivering compassionate support in a 100% remote environment, with a Monday–Friday schedule and no weekends. Prior health coaching or patient support experience is beneficial.

Qualifications

• High School Diploma or GED.
• Strong communication and interpersonal skills.
• Ability to motivate, encourage, and support patients.
• Excellent organizational and multitasking abilities.
• Comfort using computers, web-based platforms, and remote technology.
• Preferred degrees in psychology, counseling, or health-related fields.
• Experience in health coaching, patient care coordination, or customer service.

Responsibilities

• Provide education and coaching to patients using CPAP therapy.
• Encourage long-term therapy adherence through motivation, accountability, and support.
• Conduct outbound follow-up calls to patients and caregivers.
• Respond to inbound patient and caregiver inquiries.
• Review daily patient activity in the portal (reporting only).
• Identify barriers to therapy success and coordinate with care teams.
• Schedule and reschedule patient appointments as needed.
• Document all patient interactions accurately in electronic systems.
• Participate in team meetings, training, and quality initiatives.
• Remain available and engaged during scheduled work hours.
