Job summary

Conviva Senior Primary Care, a Humana-affiliated organization, seeks a Clinical Pharmacist to support a physician panel across multiple clinics in a remote role. You will apply evidence-based pharmacotherapy, optimize drug coverage, and participate in high-risk rounds with a multidisciplinary team.

Requirements include a PharmD, ambulatory care residency, and ideally board certification in geriatric pharmacy, with proficiency in EMR systems and bilingual English/Spanish.

Qualifications

• Doctor of Pharmacy degree or equivalent required.
• Ambulatory care residency or similar training preferred.
• Board certification in geriatric pharmacy preferred.
• Experience in ambulatory care/primary practice settings.
• Familiarity with EMR systems.
• Multilingual (English/Spanish) preferred.

Responsibilities

• Evaluate pharmacotherapy for high-risk patients with chronic conditions.
• Execute clinical protocols per organizational and regulatory guidelines.
• Optimize drug coverage and formulary use to reduce costs.
• Participate in high-risk rounds with a multidisciplinary team.
• Provide patient counseling and clear health information to patients and providers.
• Document all activity in EMR and other systems.
• Support CMS Stars and HEDIS quality measures.
• Conduct staff education on pharmacotherapy.
