Job summary

Humana is seeking a Clinical Pharmacist to join our remote primary care team. You will leverage evidence-based pharmacotherapy to optimize care for high-risk patients with chronic conditions, including diabetes and hypertension, while collaborating with a multidisciplinary team.

Qualifications

• PharmD or equivalent degree.
• Residency in ambulatory care.
• Board certification in geriatric pharmacy.
• Experience in ambulatory/primary care setting.

Responsibilities

• Evaluate pharmacotherapy for high-risk patients with chronic conditions.
• Collaborate with a multi-disciplinary team and participate in rounds.
• Document activity in EMR and support CMS Stars and HEDIS.
• Provide patient counseling and education on medications.
• Optimize drug coverage and reduce costs.
