Job summary

Conviva Senior Primary Care seeks a Clinical Pharmacist to join a remote primary care team. You will improve patient outcomes through medication management, patient education, and collaboration with the healthcare team, supporting the full physician panel across locations.

You will report to the Associate Director Integrated Clinic Pharmacist and participate in multidisciplinary rounds, with a focus on ambulatory care and quality improvement, while ensuring regulatory compliance and excellent

Qualifications

• Active and unrestricted pharmacist license in Florida.
• Willingness to become licensed in additional states for market support.
• Pharm.D. or other advanced pharmacy degree.
• Minimum five (5) years of experience as a practicing pharmacist.
• Experience working in a team environment and supporting common goals.
• Proficiency using MS Office programs (Word, Excel, PowerPoint, Teams, Outlook) and basic remote-technical troubleshooting.
• Multilingual (preferably English/Spanish) and ability to pass language assessments.

Responsibilities

• Evaluate pharmacotherapy for high-risk patients with chronic conditions using evidence-based medicine.
• Execute clinical protocols per organization and state regulations.
• Optimize drug coverage and reduce costs with preferred formulary products.
• Collaborate with physicians, nurses and other providers to optimize care.
• Participate in high-risk rounds with multidisciplinary teams.
• Counsel patients and communicate complex medical information clearly.
• Educate patients on disease states and self-management.
• Engage in quality improvement to enhance safety and efficacy.
• Perform post-discharge medication reconciliation for high-risk patients.
• Document activity in EMR and management systems.
• Support CMS Stars and HEDIS quality measures.
• Maintain documentation of interventions and patient care activities.
• Provide in-service education and training to staff.
