Job summary

Humana Conviva Senior Primary Care seeks a Clinical Pharmacist to support the full physician panel across multiple clinic locations in a remote role, reporting to the Associate Director Integrated Clinic Pharmacist and integrating the pharmacist as a member of the care team within the physician practice.

Qualifications

• Proficiency using MS Office programs such as Word, Excel and Power Point, Teams, Outlook.
• Multilingual (preferably English/Spanish) - Must be able to pass both language assessments.
• Clinical residency in ambulatory care or similar discipline.
• Board certification in geriatric pharmacy.
• Experience working in an ambulatory care/primary practice setting.
• Familiarity with electronic medical record (EMR) systems.

Responsibilities

• Support the full physician panel and integrate the pharmacist into the care team.
• Use evidence-based medicine to evaluate pharmacotherapy for high-risk patients.
• Execute clinical protocols per organization and state regulatory agencies.
• Optimize drug coverage benefits and reduce costs with formulary and home delivery options.
• Participate in high-risk rounds with a multi-disciplinary team.
• Provide excellent patient counseling and communicate complex medical information.
• Engage in quality improvement initiatives for medication safety and efficacy.
• Post-discharge medication reconciliation for high-risk transitions of care.
• Document activity within clinic EMR and other required systems.
• Support CMS Stars and HEDIS quality measures.
• Conduct in-service education and develop educational materials.
• Address initiatives directed by leadership and regulatory standards.
