Humana Inc. seeks a Clinical Pharmacist with 5+ years of experience to join our primary care organization remotely in Florida. You will optimize medication management, provide patient education, and collaborate with a care team to improve outcomes.

This role requires strong clinical judgment, critical thinking, and excellent patient counseling. You will document activities in the EMR, support quality measures, and participate in multidisciplinary rounds.

Qualificações

• Active and unrestricted pharmacist license in Florida.
• Pharm.D. or advanced pharmacy degree.
• Stellar collaboration within a healthcare team.

Responsabilidades

• Evaluate pharmacotherapy for high‑risk patients with chronic conditions using evidence‑based medicine.
• Execute clinical protocols as allowed by organization and state agencies.
• Collaborate with physicians, nurses and the team to optimize patient care.
• Participate in high‑risk rounds with a multidisciplinary team.
• Document all activities in clinic EMR and required systems.
• Support CMS Stars and HEDIS quality measures for clinic performance.
