Humana's Conviva Senior Primary Care is seeking a Clinical Pharmacist with 5+ years of experience to work remotely within the Florida market. You will optimize pharmacotherapy, educate patients, and collaborate with physicians and nurses to improve outcomes.

The role requires strong clinical judgment, post-discharge reconciliation, and participation in quality initiatives. Residency in ambulatory care preferred, and you must reside within 50 miles of Okeechobee or Lake Worth, FL.

Qualifikationen

• Active and unrestricted pharmacist license in the state of Florida (or eligible).
• Pharm.D. or equivalent required with 5+ years of practicing pharmacist experience.
• Experience collaborating with a healthcare team and providing patient education.
• Proficiency with MS Office and remote workstation troubleshooting.

Aufgaben

• Evaluate pharmacotherapy using evidence-based medicine and clinical judgment for high-risk patients.
• Collaborate with physicians, nurses and staff to optimize patient care.
• Lead post-discharge medication reconciliation and ensure accurate documentation in EMR.
• Support CMS Stars and HEDIS quality measures and participate in quality improvement initiatives.
• Provide in-service education and develop educational materials for staff.
• Communicate complex medical information to patients and providers.
