Conviva Senior Primary Care seeks a Clinical Pharmacist to improve patient outcomes via medication management and patient education. This remote role reports to the Associate Director Integrated Clinic Pharmacist and works as part of the care team across multiple clinics. You will support high‑risk patients with chronic conditions and collaborate with physicians, nurses, and staff.

Qualified candidates have a Pharm.D. or higher, a Florida license, and at least five years of practice experience.

Qualifikationen

• Active and unrestricted Florida pharmacist license.
• Pharm.D. or other advanced pharmacy degree.
• Minimum five years of practicing pharmacist experience.
• Experience in team-based clinical care and remote work setup.
• Proficient with MS Office and basic tech troubleshooting.
• Willingness to obtain additional state licenses for market support.

• Evaluate pharmacotherapy for high-risk patients using clinical judgment.
• Execute clinical protocols per organization and state rules.
• Optimize drug coverage and reduce costs with formulary guidance.
• Collaborate with physicians, nurses and health professionals to optimize care.
• Participate in high-risk rounds with a multidisciplinary team.
• Provide patient counseling and education on medication use.
• Engage in quality improvement to improve safety and efficacy.
• Perform post-discharge medication reconciliation for high-risk patients.
• Document activity in EMR and clinical management systems.
• Support CMS Stars and HEDIS quality measures in clinic performance.
• Deliver in-service education and create pharmacotherapy materials.
• Stay current with pharmacotherapy advances and guidelines.
• Consult with providers on complex medication management issues.
