Job summary

Humana Inc in the Conviva Senior Primary Care family seeks a Clinical Pharmacist to join a remote team improving patient outcomes through medication management and patient education. You will support the full physician panel and collaborate with care teams across clinic locations.

Responsibilities include optimizing drug coverage, participating in high-risk rounds, documenting in EMR, and advancing CMS/quality initiatives. Requires Pharm.D.

Qualifications

• Active and unrestricted pharmacist license in the state of Florida.
• Willingness to become licensed in additional states for market support.
• Pharm.D. or other advanced pharmacy degree.
• Minimum five (5) years of experience as a practicing pharmacist.
• Experience working in a team environment and supporting common goals.
• Proficiency using MS Office programs such as Word, Excel and Power Point, Teams, Outlook, and troubleshoot and resolve basic technical needs within a remote environment.
• Multilingual (preferably English/Spanish) - Must be able to pass both language assessments.

Responsibilities

• Use evidence-based medicine and clinical judgment to evaluate pharmacotherapy for high-risk patients.
• Execute clinical protocols as allowed by organization and state agencies.
• Optimize drug coverage benefits and reduce costs.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Participate in high-risk rounds with a multidisciplinary team.
• Provide excellent patient counseling and education.
• Engage in quality improvement to enhance safety and efficacy.
• Document all activity in EMR and systems.
