Qualifications

• Active and unrestricted pharmacist license in Florida.
• Minimum five (5) years of experience as a practicing pharmacist.
• Experience working in a team environment and supporting common goals.
• Proficiency with MS Office programs and remote work tools.
• Willingness to obtain licenses in additional states.

Responsibilities

• Evaluate pharmacotherapy for high-risk patients using evidence-based protocols.
• Collaborate with physicians, nurses and other professionals to optimize care.
• Participate in high-risk rounds and adjust treatment plans.
• Provide patient counseling and education on medications and self-management.
• Document clinical interventions in EMR and ensure regulatory compliance.
• Support CMS Stars and HEDIS quality measures.

Skills

Multilingual - English/Spanish

Education

Pharmacist license in Florida

Willing to obtain additional state licenses

Pharm.D. or other advanced pharmacy degree

Clinical residency in ambulatory care or similar

Board certification in geriatric pharmacy

Experience in ambulatory care/primary practice setting

Familiarity with EMR systems
