Sagility is seeking a seasoned Registered Nurse to join as Senior Care Specialist in a work-from-home capacity. You will assess members remotely, develop care plans, provide education, and coordinate services to improve health outcomes for Medicare, Medicaid, and group health plan members.

This seasonal role offers a competitive starting wage of $37 per hour, with potential bonuses and incentive programs after joining Sagility.

Qualifikationen

• Active, unrestricted RN license in home state.
• Active Compact RN license.
• Minimum 2 years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency including MS Word, Excel, and Outlook.
• Ability to navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Aufgaben

• Conduct telephonic health assessments for members.
• Develop actionable, individualized, and measurable care plans.
• Establish health coaching relationships and provide education and support.
• Use nursing judgment to assess needs and coordinate interventions.
• Support transitions of care and discharge planning, including DME and home health services.
• Help members navigate healthcare benefits and care options.
• Assess physical, environmental, and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams to support care.
• Promote self-management with education and resources.
• Coordinate care among members, caregivers, providers, and care team.
• Document interactions, assessments, and outcomes accurately in systems.
