SCAN Health Insurance is hiring a remote Care Management Team Leader to oversee the day-to-day operations of a multidisciplinary team serving seniors enrolled in Medicare/Medicaid programs. The role emphasizes regulatory compliance, care quality, and coordinated member support across telephonic and field staff.

The position requires 2+ years in managed care or case management, strong leadership, and excellent communication. Most work is remote with occasional on-site collaboration as needed.

Formación

• Bachelor's degree or equivalent experience.
• 2+ years experience in managed care, healthcare environment, or case management.
• 2+ years working remotely strongly preferred.
• Bilingual ability strongly preferred.

Responsabilidades

• Oversee day-to-day operations of a multidisciplinary care management team.
• Define daily priorities for telephonic and field staff and ensure regulatory compliance.
• Provide performance updates and coaching through regular meetings.
• Resolve escalated issues with members, providers, and external departments.
• Develop and oversee chronic disease management programs.
• Maintain policy and procedure alignment with regulatory standards.
• Lead team meetings and special projects balancing priorities.
• Utilize motivational interviewing to address member concerns and de-escalate when needed.
