SCAN Health Insurance seeks a Clinical Care Coordinator to empower older adults to age in place by coordinating behavioral health and medical services. The role emphasizes compassionate care navigation, referrals, and seamless communication with members and providers.

The candidate should have extensive experience in managed care, strong motivational interviewing skills, and the ability to work remotely with minimal supervision. A bilingual Spanish background is strongly preferred.

Competenze

• 5+ years working in managed care, healthcare environment, or behavioral health case management.
• 2+ years working with seniors and remotely preferred.
• Bilingual Spanish strongly preferred.
• Advanced technical skills for functional area.
• Strong interpersonal and organizational skills; critical thinking.

Mansioni

• Perform general and behavioral health screenings, identify referrals, and support timely intake.
• Serve as a point of contact for members, providers, and care teams regarding benefits and services.
• Utilize motivational interviewing to address member goals and de-escalate when needed.
• Coordinate care with internal and external teams to develop action plans and resolve access barriers.
• Document interactions and care plans in EMR and tracking systems within deadlines.
• Educate providers on contract requirements and navigate authorization barriers.
