Oak Street Health is seeking a Clinical Call Center Triage Nurse for a remote, full-time role. You will extend care beyond clinic hours by addressing patients' medical, social, and psychological needs via telephone, while documenting accurately and supporting appointment scheduling and transportation needs.

Key qualifications include an active RN license, reliability, and a patient-centered approach. Flexibility and US work authorization are required to succeed in this role.

Qualifications

• Active, non-probationary Registered Nurse license in all OSH practice states.
• Ability to manage time and work autonomously with reliability.
• High level of conscientiousness and focus on patient experience.
• Willingness to work flexible shifts including evenings and weekends.
• US work authorization.

Responsabilités

• Provide confident, professional service to OSH patients.
• Follow prescription refill policy and document clearly during calls.
• Schedule appointments and arrange transportation as needed.
• Complete documentation for all patient requests before ending a call.
• Monitor CareReqs and complete tasks within scope.
• Support remote training and participate in remote work requirements.
