Job summary

Call 4 Health is seeking an experienced Registered Nurse to join our 24-hour remote triage team. The RN will conduct phone and portal assessments, document encounters, and support providers across multiple states from a home office.

Qualified candidates hold an Associate's or Bachelor's in Nursing, 2–4 years of full-time RN experience (ER/urgent care or similar), a Compact licensure, and experience with two or more EHR systems.

Qualifications

• RN degree or higher and valid licensure.
• 2–4 years of full-time RN clinical experience (ER/urgent care or similar).
• Experience with telephone triage using electronic triage systems.
• Experience with two or more EHR platforms.
• Active, unencumbered Compact licensure (e-NCL).
• Willingness to obtain additional state licenses as needed.

Responsibilities

• Respond to inbound/outbound calls for multi-state triage service.
• Use triage protocols to assess and triage patients.
• Document encounters accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative/clinical tasks and referrals.
• Coordinate prior authorizations and post-discharge follow-ups.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in RPM programs and educate patients on chronic disease management.
• Screen and qualify patients for clinical trials and education.
• Take on extra duties to support team and outcomes.
