Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) to join our 24/7 remote triage center. You’ll handle telephone triage, EHR management, and clinical support across multiple states from a home-based setting.

The role emphasizes clinical decision-making, patient education, and collaboration with providers to deliver timely care. A compact nursing license is required, with flexibility for additional state licenses as needed.

Qualifications

• Active nursing license in good standing.
• 2–4 years of RN clinical experience.
• Experience with telephone triage and triage protocols.
• Experience with two or more EHR platforms.
• Compact license (eNLC) or willingness to obtain additional state licenses.

Responsibilities

• Respond to inbound/outbound calls for a multi-state nurse triage service.
• Utilize established triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks including med refills and notifications.
• Coordinate post-discharge and post-operative follow-up with patients.
• Facilitate referrals and assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, and government agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs and educate patients on disease management.
• Take on additional responsibilities to support team success and patient outcomes.
