Job summary

Call 4 Health is seeking a Registered Nurse to join its remote triage team, delivering high-quality patient assessments from home to clients nationwide. You’ll triage calls, document in EHRs, and collaborate with providers to ensure timely care.

Ideal candidates hold an Associate’s or Bachelor’s in Nursing with 2–4 years of RN experience, including telephone triage and multiple EHR systems. Active e-NCL license required; willingness to obtain additional state licenses as needed.

Qualifications

• RN with Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience.
• Prior telephone triage experience with electronic triage systems.
• Experience using two or more EHR platforms.
• Active, unencumbered e-NCL/ Enhanced Nurse Licensure Compact license.

Responsibilities

• Respond to inbound/outbound calls for a multi-state nurse triage service.
• Use established protocols to assess and triage patients.
• Document interactions in client EHR systems and/or Call 4 Health platform.
• Provide clinical assessments via phone or patient portal.
• Support providers with admin/clinical tasks (refills, critical results, follow-ups).
• Coordinate referrals and prior authorization with clinical input.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs.
• Screen and educate patients for clinical trials.
