Call 4 Health is looking for a Registered Nurse (RN) for Remote Triage, requiring a Compact License. You will assess and triage patients by telephone, document in EHRs, and support providers across multiple states from a home office.

Qualifications

• Associate's or Bachelor's Degree in Nursing required.
• 2-4 years of full-time RN clinical experience, ER/urgent care, adult/pediatric or ICU preferred.
• Prior telephone triage experience using electronic triage systems.
• Experience with two or more EHR platforms.
• Active, unencumbered Compact license; willingness to obtain additional state licenses.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize telephone triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks, including medication refills, provider notifications, and follow-up coordination.
• Facilitate referrals and assist with prior authorization requests requiring clinical input.
• Collaborate with hospitals, nursing homes and other agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs and educate patients on disease management.
• Screen and qualify patients for clinical trials and take on additional responsibilities as needed.
