Job summary

Call 4 Health seeks a Remote Nurse Triage RN to support patients nationwide from a home-based setup. You will conduct telephone triage, document in EHRs, and provide clinical assessments following established protocols, while assisting providers with referrals and follow-up tasks.

Ideal candidates have 2–4 years RN experience, an active Compact license, and a nursing degree; willingness to obtain extra state licenses. This full-time remote role follows ET schedules and a fast-paced environment.

Qualifications

• Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience in ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Active, unencumbered e-NCL / Enhanced Nurse Licensure Compact license.
• Willingness to obtain additional state licenses as requested.

Responsibilities

• Respond to inbound/outbound calls for a multi-state nurse triage service.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks, including refills and referrals.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs (vital signs, chronic disease education).
• Screen and qualify patients for clinical trials and patient education.
