Job summary

Call 4 Health is seeking experienced Registered Nurses to join our 24-hour remote triage team. As a Remote Triage Nurse, you’ll provide telephone triage, clinical assessments, and EHR documentation for patients across multiple states.

This role offers flexible shifts in ET, requires an active compact license, and prior triage or urgent care experience. You will collaborate with providers, support referrals, and participate in RPM programs from a HIPAA-compliant home workspace.

Qualifications

• Active RN licensure with a current license in good standing.
• 2–4 years of full-time RN clinical experience, preferably ER/urgent care or ICU.
• Prior telephone triage experience using electronic triage systems.
• Willingness to obtain additional state licenses as requested.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize established protocols to assess and triage patients via phone.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal following protocols.
• Support providers with administrative and clinical tasks, including medication refills.
• Notify providers of critical results and coordinate post-discharge follow-ups.
• Facilitate referrals and assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, and agencies as needed.
• Participate in remote patient monitoring, including vital signs and patient education.
• Screen and qualify patients for clinical trials and provide patient education.
